Player Registration

• Step 1: Fill out and sign the Orca Youth Rugby Registration Form
• Step 2: Complete appropriate USA Rugby Waiver and Consent form:
  • a) USA Rugby Waiver and Consent Form (Minor Under 18)
    or
  • b) USA Rugby Waiver and Consent Form (Adult 18 or older)
• Step 3: Make copies (no originals please):
  • a) Birth certificate(returning players also)
  • b) Medical Insurance Card (both sides)
  • c) School Photo ID or Drivers License
• Step 4: Fill out short Survey Questionnaire
• Step 5: Write check payable to “Orca Youth Rugby” for $170. Additional siblings are $150 each. (Online payment for registration is coming soon!)
  (Note: If you cannot afford to pay the full amount now, write a check for $40 and we will contact you to set up a payment plan. We do not want money to be an issue to keep a youth from playing rugby.)
• Step 6: Mail all of the above to: Orca Youth Rugby, 2478 Ellington Court, Simi Valley, CA 93063
